APH Consumables and Capital Equipment Forms Going Paperless

We are excited to announce that our APH Consumables and Capital Equipment Loan Forms are going paperless and are being combined into one online form!

Starting on March 1, if you would like to request APH Consumables, loan of an APH Device (Chameleon, Jupiter, etc.) or at AT Capital Equipment Loan (BrailleNote, BrailleSense, etc.) please utilize our new online form!

The form can be accessed from the WCBVI AEM Webpage or by following the link below.

WCBVI Outreach will be holding an Open House on March 11th at 2:00. Please register using the link below. After registering, you will receive a confirmation email containing information about joining the meeting.

Video walkthroughs and training on how to fill out the forms will be posted to the WCBVI YouTube channel.  Please watch the Weekly Update for announcements when these options are available.

Q. Why has WCBVI created an online request form for APH materials and loan items?

A. WCBVI is committed to creating a streamlined and accessible process for submitting these requests. We hope that an online submission process will expedite turnaround time and make the request process easier for all involved.

Q. Will this form be accessible to those using a screen reader?

A. YES! We are very excited that this form will be fully accessible to individuals using screen readers.

Q. Will I be able to submit PII (Personally Identifiable Information) such as student names using this form

A. YES! Microsoft Forms are approved for transmission of personally identifiable information (PII) and are Family Education Rights Privacy Act (FERPA) compliant.

Q. Does an LEA still need to sign for loan of equipment?

A. An LEA’s signature is not required for all loan items. It will be needed for braillewriters and any AT Loan Items not tied to APH Quota such as braille notetakers.

Q. How will the LEA sign the form?

A. Once the requestor submits the Microsoft Form, a separate form will be initiated via DocuSign. The TVI, LEA, and EOT (Dawn Soto) will all need to sign the form electronically before equipment can be shipped.

Q. Can I still submit a hard copy form via Fax or US Mail?

A. Stakeholders may submit hard copy forms until June 17, 2022. After that, all requests must be made using the online form.
