WCBVI APH Consumables and Capital Equipment Procedures

ALERT: COVID-19 and PROCEDURAL CHANGES

Following the Safer at Home guidance from Governor Evers, our WCBVI Outreach staff are working remotely without access to our physical building.

As we do not have normal operations onsite at this time, we’re unable to accept APH Consumable and Capital Equipment Loan Request Forms via fax, email, scan, or other electronic formats.

To provide safe transmittal of Personally Identifiable Information (PII) and reduce risk, we have implemented several important changes.

REMINDER: To comply with FERPA/HIPAA governance and PII, we are unable to accept student names online. For information regarding FERPA/HIPAA, please visit The US Department of Education FERPA Guidelines for Students

General APH Guidelines

Please note that to receive APH consumable materials, or borrow equipment purchased with APH Quota funds, (including high cost technology) the following requirements must be met:

The student must be currently on the APH registry

The student must have a FERPA waiver on file with the APH Census Coordinator

Semester loans are due back the third Friday in December and the third Friday in May.  Please consult the Loan Agreement form you receive with your equipment for the exact date

Long term loans must be returned to WCBVI when the student leaves the district, leaves the state, or is no longer eligible for special education due to aging out or graduation.

APH Consumables

To request APH consumable items the student must be currently registered on the APH Census and have a FERPA/ROI on file with the Wisconsin APH Census Coordinator. APH Consumable items do NOT need to be returned to WCBVI.

APH Consumables requests will be placed in a pending status until our onsite operations resume.

The APH Consumable Request Form must be submitted via USPS or received over the phone. The two options available are as follows:

Submit via the USPS mail (no fax or email submittals allowed):

  1. Print and ensure all information is legible and all fields are completed, including the APH Catalog Number, which can be found online with APH at: https://www.aph.org/shop/
  2. MAIL to:
    WCBVI Outreach
    Attn: APH Consumable Request
    1700 West State Street
    Janesville, WI 53546

Submit via phone if you don’t have access to the USPS:

  1. Ensure you have all student information ready to submit over the phone
  2. Ensure you know APH Catalog Numbers of items, which can be found online with APH at: https://www.aph.org/shop/
  3. Phone our Outreach Support Specialist at (715) 612-2003

APH High Cost Technology Loan Procedure

APH High Cost Technology Loan requests will be placed in a pending status until our onsite operations resume.

To borrow technology in our APH Loan Library, Capital Equipment Loan Agreement Request Form must be submitted via the USPS.

Submitted via the USPS mail (no fax or email submittals allowed):

  1. Print and ensure all information is legible and all fields must be completed, which includes the signature of the special education director/LEA. 
  2. MAIL to:

    Attn: Capital Equipment Request
    1700 West State Street
    Janesville, WI 53546

WCBVI Category 1 APH Loan Items

The following items are marked as Category 1 Loan Items because they are not advanced technologies and don’t require regular updates.
